Welcome to Family Faith Formation and Sacramental Preparation at St. Mary's!


 If your child was PREVIOUSLY ENROLLED in our Faith Formation classes, please only answer
 "Yes" or "No" for his/her "Sacraments". You do not need to fill in a date or a location.

 If your child is a NEW student, please complete the "Sacraments" section, including dates and
 location. In addition, we will need a copy of your child's baptismal certificate.

 To avoid confusion, please complete the "Head of Household" section with the information of the
 person in your household who should receive all registration communication.

 Payment is required at the time of registration. You will receive a system-generated email with
 online payment instructions, or other ways to pay. Our office will send you another email which
 will include an emergency contact sheet and waivers that we need to have completed and on file
 for each student.

 Your registration will not be complete and students will not be assigned to a class until this
 information is returned and a full or partial payment is made!

 Please complete this prior to August 31 as we anticipate classes will fill quickly due to class size
 limits.

Form's Instructions If you request a Sacrament of Initiation (Baptism, First Communion, Confirmation) for a student in addition to Faith Formation class, we will contact you for placement.
All new students must submit a copy of their Baptism Certificate if not baptized at St. Mary's.
All new students in grades 8-12 will be placed in an Confirmation Preparation Year 1 class.
